Required Registration Information
If paying online or in person all returning and new families must complete a new registration packet for the current academic year and return all forms with original signatures during the following appropriate registration period. The registration forms may not be faxed or emailed.

The Registration Packet can be downloaded from the School website and consist of the following four forms: registration, medical info, media release and Guidelines signature page.

Registration for Service Starting August 13, 2012 is August 1- 3 & August 6^th  5:00 to 7:15 p.m. at the UpperAcademy main building reception area

Year-Round Registration is only allowed on the 23 - 25 days of the month prior to the month of service, 4:30 to 6:00 p.m. at the UpperAcademy main building reception area.

Enrollment Confirmation
Participant’s slot in the Program will not be held until all fees are paid and the properly completed registration forms are received by the Program manager or administrative assistant.
